NEXT PHYSICAL DATE SET:Tues. July 29, 2008,COST: $15.00. Checks made payable to: Dr. Sharon Siefert. LOCATION: APPLEWOOD FAMILY MEDICINE, 542 S. STATE ST.(BY THE SECRETARY OF STATES OFFICE) Athletes should line up outside of the building at 4:30 p.m. Doors will open at 5:00 p.m. (Everyone must register by 6:00 p.m., doors will close at theat time.) Physical forms are available in the Main office (Summer hours are 7:00 A.M. – 12:00 and 1:00 p.m. – 3:00 p.m. Monday thru Thursday) Be sure your forms are filled out and signed by a parent before getting in line!
